When people need more support to manage their day to day lives, it can be helpful to hire a Case Manager. A Case Manager is a subject matter expert who helps to manage systems, link people to care, assist with benefit acquisition, and shares community resources. This resources may include housing, complex physical and mental health, legal and financial needs. A Case Manager often helps people (and their support systems) who experience severe and persistent mental illness, including: (severe) Depression, Bipolar, Schizophrenia, Schizoaffective Disorder and other forms of Psychosis.
